When you call Child Guidance & Family Solutions 330-762-0591 to make an initial appointment or make a referral, an experienced intake specialist is ready to help and answer your questions. They will gather information about your concerns and schedule your appointment. If we are not the best fit for you, then we will provide referral information.

What to expect at your first appointment?

Completion of Paperwork- Client Questionnaire that includes medical and family history

Financial - Please bring your Insurance/Medicaid cards(s) and client’s social security number. Summit County residents can access a reduced/sliding fee based on family size and monthly income. Bring documentation of 1 month’s family income.

Assessment - An experienced behavioral health professional will talk with you to understand the challenges you face. A Diagnostic Assessment Report is prepared that details information you have provided and is then used to focus the treatment needed.
